  • Our complex care programs are designed to improve care and outcomes for our most medically complex members. Clover wants to take accountability for these members' healthcare journeys and provide high-quality personalized care that is consistent with members' values and preferences.
  • The Wellness Nurse Care Manager collaborates closely with members, their health providers, and Clover clinical teams to improve member health outcomes. To do so, the WNCM facilitates member assessments, evaluations, care planning, care coordination, and advocacy related to a member’s comprehensive health needs. This process includes medical, social, developmental, behavioral, financial, and educational intervention with focus on high quality and cost effective outcomes. The Wellness Nurse Care Manager will report directly to the program lead.

    As a Wellness Nurse Care Manager, you will:

  • Engage and provide telephonic care coordination and management to identified patient populations.
  • Effectively collaborate with members of Clover clinical teams (e.g. In Home Care team, Readmission Prevention Program team, Supportive Care team, Behavioral Health team, field Nurse Practitioners, Medical Assistants).
  • Initiate referrals to specialty services, follow up on open referrals, and foster continuity of care.
  • Create a personalized care plan for each member to resolve barriers to care and engage with social support systems as appropriate.
  • Follow NCQA Care management accreditation standards.
  • Document all member care activities and escalate findings in accordance with Clover Policies.
  • Coordinate with the interdisciplinary team as needed, including members’ outpatient providers.
  • Provide telephonic teaching and counseling in the areas of health promotion, disease prevention, maintenance, and management of acute/chronic diseases.
  • Attend and participate in regular ongoing meetings.
  • Demonstrate flexibility in assignments within Clover’s Clinical Programs based on member and company needs.
  • Advocate for members to ensure their needs and choices are fully represented and supported.
  • Participate in ongoing professional development and self-improvement.

As a Patient Care Coordinator (PCC), you will be the central point of contact and communication for the primary care provider (PCP), medical assistant (MA), patients, caregivers, and external agencies. As a Patient Care Coordinator, you will:

We are hiring our first Product Operations Manager on the Counterpart Assistant team to help improve the operating model of our pods and product organization. As a company, we employ an empowered pod model for our technology team - putting pods in the center of solving for business and user outcomes. With that model, we’ve been able to scale with fewer processes and systems explicitly managed, but those soft systems are in need of support. The Product Operations Manager will work closely with our VP of Product and our Counterpart Technology leadership team to help reinforce those soft systems to enable more efficient pod and company velocity.

Product operations can have a wide range of framings, at Counterpart the role description with which we most resonate is that of Melissa Perri or Marty Cagan, the The Force Multiplier Model for Product Operations.

The ideal candidate will be data and process oriented, who thrives on figuring out how to more effectively empower rapid iteration and learning and consistently like automating themselves out of a job.

As a Product Operations Manager, you will:

  • Work alongside product managers, designers, engineers, data scientists, researchers, clinical and all of our interdisciplinary pod teams to figure out where we have inefficient cross-team systems.
  • Focus on identifying and solving the highest impact process enhancements for our pods, stakeholders and our leadership team.
  • Be a collaborator with pods in developing KPIs that ladder up to our OKRs for the year and specifically measure the work being shipped in a meaningful way for our users and business.
  • Support and integrate with our user research, customer success, product support, sales, and clinical teams to make sure teams have access to and are engaging with customers regularly and accessing those insights actively.
  • Partner closely with our VP of Product to coalesce roadmaps, status updates, run projects, and deliver improved pod operations. 
  • Be accountable to developing and supporting process improvement changes for our technology team’s SDLC and portfolio management to improve overall learning velocity by the pods.

Success in this role looks like:

  • Over the first 90 days you’ll spend time building relationships with the different pods, technology leadership, stakeholders, and customers to understand the current pain points in our operating model. The three focuses will be on our data and insights, voice of the customer, operating model and tooling. A known area of improvement that will be a first project is a better way for us as an organization to manage the end to end delivery and roadmap from engineering progress through product bets to high level executive roadmap(s). At each level, we have known areas of opportunity and inefficiency.
  • Within the first 6 months, success is measured as evolving the team and processes from an improved operating model. Projects should be identified in partnership with leadership, tackled and implemented, and rolled off to the next area of opportunity. As an organization we should be able to measure the efficiency of our learnings and progress and see improvement from the efficiencies of the product operations role.
  • Over time, this individual's performance will be measured as the effectiveness of our pods’ ability to iterate and learn quickly. Compensation: $125 per completed visit

As a Practitioner, you will:

  • Deliver preventive care to Clover members remotely via Clover’s proprietary software platform as well as approved third party video apps.
  • Complete a comprehensive review and assessment of a member's medical history, physical health, environment and psycho-social status.
  • Document progress notes, prepare member workups and encounter summaries using Clover Assistant and EMR.
  • Help at-risk members transition from the hospital and other facilities to their own home.
  • Create personalized care plans in coordination with the member's primary care physician.
  • Actively close gaps in care, which may include ordering labs, writing prescriptions with collaboration of the Primary Care Provider or collaborating physician, and coordinating physician follow up.
  • Perform point of care labs and diagnostic testing, such as A1c, spirometry, and other tests as applicable

    • Visit specific CPT codes added in AthenaNet Billing Tab 
    • All CA visit tasks addressed, signed and autoloaded to AthenaNet visit
  • Coordinate and communicate with members’ interdisciplinary care teams
  • Participate in ongoing training and meetings remotely
  • Comply with all Clover Care Service policies, procedures, standards of care and workflows
